Student Accessibility Forms

The following are forms for students to make requests for accommodations and/or services through the Accessibility Resources Center. All requested documentation should be submitted to ARC@newhaven.edu.


  • Students must make an Accommodation Request to request faculty be notified of approved accommodations each semester

    Students for whom electronic textbooks is an accommodation can request books they do not currently have by using the Request for Electronic Textbooks Form. Requests must be accompanied by a receipt for the purchase or rental of the book being requested.

  • The following forms are to be completed by students requesting modifications to housing and/or dining. All Students should complete the Online Housing Application.

    Students requesting Emotional Support Animals on campus should have their professional provider complete the Disability Verification Form for Emotional Support Animals.

    Students requesting specific housing configurations (e.g. single/double room, AC, accessible bathroom, etc.) should have their professional provider complete the Disability Verification for Modified Housing/Dining.

  • Students should complete the Modified Housing Renewal Form each year in January to renew their currently approved modified housing accommodations for the upcoming academic year. This form should NOT be used to make an initial request or to change an existing accommodation.

  • Complete the following form to request permission to park a vehicle on campus. This form must be submitted with documentation of the need for the vehicle including documentation of the condition, impact of the condition requiring the accommodation, and expected duration of the condition.

  • Complete the following form to request accommodations for a temporary disabling condition such as concussions, other bodily injuries, surgeries, etc. This form must also be accompanied by documentation of the condition, impact of the condition, and expected duration of the condition.
